Unsure if you ended up figuring it out, how ever I suggest reaching out to your host for your website and use pdo_dblib. Make this .php file and drop this into your root web folder -> fill out the information within that ?php script and after dropping the file into your ftp head over to your website and use that .php file it will either throw an error or will give you information from the SQL. Things to take note, make sure your webhost opens port 1433.
